Care Pathways in General Practices

Various socio-demographic and epidemiological developments have led to a significant increase in patients with chronic conditions. Managing these patients is often complex and requires a multidisciplinary care team. In practice, multidisciplinary care is frequently fragmented, resulting in a loss of care quality.

We aim to address this by developing a tool that facilitates general practices in implementing customized, integrated care pathways within their daily care organization for patients with chronic conditions. Its design will center around the vision, capabilities, and context of the primary care practice.

HALT2Diabetes

In HALT2Diabetes individuals at high risk for type 2 diabetes and cardiovascular diseases are identified using a two-step method and guided towards a healthy lifestyle to mitigate this risk. HALT2Diabetes is a project realized with the support of the Flemish government.

The first step involves administering the FINDRISC questionnaire to individuals aged 45 and older. This questionnaire is fully digitalized, making it easy to calculate the risk of diabetes and cardiovascular diseases. Based on the results, participants receive personalized advice, and can immediately send a report to the general practitioner.

For participants with a score of 12 or higher, it is important to schedule an appointment with a general practitioner to determine blood sugar levels. The general practitioner will assess the risk for cardiovascular diseases by measuring cholesterol and blood pressure, among other factors, as well.

Within JACARDI, a pilot project is designed to address shortcomings of the existing screening method while exploring how these innovations can be sustainably implemented.
