Abstract
The goal of this research is to study models of co-ordinated care in complex care situations (in Flanders and abroad), b y analysis of sources and literature search. By the increased number of chronic and invalid patients, there is also an increase of complex care situations in home care. This study will be done from the following research questions: what is a complex care situation? Which influence had a complex care situation on the persons concerned? Factors determining place of palliative care.
Abstract
Objective: to investigate the impact of some factors (i.e. complexity of social network, physical symptoms of patient, medical expenditure, organisational problems, ') upon decisions regarding place of palliative care. Design: prospective study of terminal cancer patients of 50 year and above who are expected to live one through six months. These patients as well as their main carer will be interviewed at 2 week intervals. Setting: patients's home. Abstract
The aim of the project is to evaluate the functioning of diabetes experts in local collaboration initiatives, and to improve in this way the care of diabetes type II patients. The role and tasks of the diabetes experts will be assessed. Various interdisciplinary interventions and projects are developped. MENTOR - modifying environments towards better teaching, rehabilitation, & integration.
Abstract
The MENTOR project one of 30 selected European pilot projects with the support of the European Commission DGV, within the programme "Equal Opportunities for the disabled" Its objectives are creating optimal environments fostering development and learning processes for children and adults with a developmental impairment; not apart but together with others in an inclusive environment, i.e. including children with disabilities in mainstream schools, leisure and workplace; making educators (parents, teachers etc.) more aware of their influence on development. MENTOR wants to establish a resource centre for research, intervention, counselling and documentation regarding inclusion Activities: - training for profesionals and parents; themes: enhancement of learning potential (cognitive, social and emotional)and inclusion - awareness raising via campaigns, articles, evening sessions for public, school visits - lobbying politically: participation in councils (workgroup FlemishEducation Council) - service: to parents, school teachers and team in order to realizeinclusion in mainstream schools (school visits) - elaborate and automatize a documentation centre about handicap and inclusion.Researcher(s)
