Need for co-ordination of care towards invalid or chronic patients in Flanders: an explorative study (partim: analysis of sources) 01/12/2000 - 30/11/2001

Abstract

The goal of this research is to study models of co-ordinated care in complex care situations (in Flanders and abroad), b y analysis of sources and literature search. By the increased number of chronic and invalid patients, there is also an increase of complex care situations in home care. This study will be done from the following research questions: what is a complex care situation? Which influence had a complex care situation on the persons concerned? Which models of co-ordinated care do exist? What is the effect of co-ordinated care? .
